Why cramps happen at all

This is worth saying plainly, even though it complicates the easy answer: the best available clinical trial evidence does not support magnesium as a quick fix for leg cramps. A systematic review pooling 11 randomized, controlled trials — the gold standard for this kind of question — found no meaningful benefit from short-term magnesium use, whether the cramps were pregnancy-related or unexplained. One separate, smaller trial did find magnesium oxide reduced cramp frequency after a full 60 or more days of consistent use — a real, if limited, signal that this may be a longer-game nutrient rather than something that works overnight.

A 2024 randomized, placebo-controlled trial found vitamin K2 (MK-7, 180mcg daily for 8 weeks) reduced cramp frequency, intensity, and duration in adults 65+. This specific paper underwent a formal correction by the journal in 2025 — worth knowing, though the corrected version is still cited in current clinical literature.
This is a newer, more specific finding worth knowing about — and one where being fully transparent about its history matters more than usual. Researchers conducted a multicenter, double-blind, placebo-controlled trial in 199 adults aged 65 and older, published in JAMA Internal Medicine in 2024. Participants received either vitamin K2 (as MK-7) at 180 micrograms daily, or a placebo, for 8 weeks. The K2 group showed a real reduction in how often cramps happened, how intense they were, and how long they lasted, with no significant safety concerns reported.
Here’s the transparency part: this specific paper went through a formal retraction-and-replacement process with the journal in 2025 — a legitimate part of how scientific publishing corrects itself when concerns are raised. The corrected, republished version continues to be referenced by clinicians and researchers in current literature as of 2026, which suggests the core finding held up through that correction process. Still, this is an emerging, single-trial finding rather than settled science, and it’s also worth knowing the studied dose (180mcg) is higher than what’s typically found in a daily multivitamin — so it shouldn’t be assumed that any K2-containing product automatically replicates this specific result.
